We had the same problem. For smaller sites - I really like to recommend 1shoppingcart.com
But if you have very specific needs - you'd need to start with an open source system and hire some programmers to customize it to your needs.
We build are own carts from scratch using Cake PHP. But it's by no means easy. We've had to programmers working on it full time for 5 months now it's behind schedule. But it's the price you'll have to pay if you have a rigid set of requirements.
Would you consider selling the cart your programmers have created when it is good to go? I think that would be a nice opportunity for you if this is a problem for internet marketers...
